Diversidades y acceso a la salud: barreras que enferman?

Abstract

En el marco del Proyecto de investigación “Salud integral y géneros: accesibilidad, producción de subjetividad y cuidados en Hospital Provincial de La Plata” perteneciente a las Facultad de Psicología de la UNLP, bajo la dirección de Claudia Orleans, surgieron algunos interrogantes que constituirán las líneas fundamentales que se pretenden desarrollar en este artículo. El proyecto de investigación se propone, entonces, tratar de analizar las narrativas de los/as trabajadores/as de un Hospital Público Provincial de la ciudad de La Plata, a partir de entrevistas. En éstas últimas intentaremos explorar qué significaciones imaginarias sociales y qué relaciones de poder se ponen en juego, formando una barrera invisible que obstaculiza el acceso al derecho a la salud de personas LBGTIQ+, en términos también de lo que podemos definir como violencia institucional y/o simbólica. Así mismo, el presente trabajo tiene como objetivo pensar una articulación-análisis de las injerencias en las diversidades sexuales respecto a la accesibilidad al sistema de salud. Cabe aclarar que encontrándonos en la primera fase del desarrollo del proyecto estamos comenzando con la revisión y exploración tanto del marco teórico-conceptual cómo de estudios o investigaciones previas. Esta investigación exploratoria-descriptiva, se desarrolla a partir de lo que se considera un enfoque cualitativo que piensa a la investigación como una práctica situada, activa, reflexiva y dialogada entre las/os investigadoras y las/los participantes. Las características del problema de investigación planteado, supone que metodológicamente se incorpora la cuestión del significado y la interpretación como inherentes a los actos, estructuras y relaciones sociales (Denzin y Lincoln, 2011, 2012). La estrategia interpretativa adoptada, se posiciona desde la hermenéutica crítica. Queremos decir que implica considerar una interpretación de las narrativas singulares, los procesos subjetivos e intersubjetivos en relación a las dimensiones histórico sociales en las que se inscriben. Es decir, para desentrañar el sentido común de su artificialidad (Arensburg et. al., 2013; Ricoeur, 2000), lo que incluye aspectos de inconclusividad e indexicalidad propios del proceso de creación y crítica del conocimiento (Parker, 2007). Nos interesa señalar que son varias las investigaciones en el sector salud que han hecho foco en indagar la relación/vinculación de las personas usuarias con la institución salud, abriendo camino al estudio de la accesibilidad centralmente en sus dimensiones: geográfica, económica, administrativa, simbólica y relacional (Stolkiner, A. et al 2008) al entender que constituye una práctica social del sistema que subyacen a la garantía de derechos.

Within the framework of the research project “Comprehensive health and gender: accessibility, production of subjectivity and care in the Provincial Hospital of La Plata” belonging to the Faculty of Psychology of the UNLP, under the direction of Claudia Orleans, some questions arose that will constitute the fundamental lines that are intended to be developed in this article. The research project aims, then, to try to analyze the narratives of the workers of a Provincial Public Hospital in the city of La Plata, based on interviews. In the latter we will try to explore what imaginary social meanings and what power relations are put into play, forming an invisible barrier that hinders access to the right to health of LBGTIQ+ people, also in terms of what we can define as institutional violence and/or symbolic. Likewise, the objective of this work is to think about an articulation-analysis of interferences in sexual diversities with respect to accessibility to the health system. It should be noted that, being in the first phase of the development of the project, we are beginning with the review and exploration of both the theoretical-conceptual framework and previous studies or research. This exploratory-descriptive research is developed from what is considered a Qualitative Approach that thinks of research as a situated, active, reflective and dialogued practice between the researchers and the participants. The characteristics of the research problem posed suppose that methodologically the question of meaning and interpretation is incorporated as inherent to social acts, structures and relationships (Denzin and Lincoln, 2011, 2012). The interpretive strategy adopted is positioned from a critical hermeneutics. We mean that it implies considering an interpretation of the singular narratives, the subjective and intersubjective processes in relation to the historical-social dimensions in which they are inscribed. That is, to unravel the common sense of its artificiality (Arensburg et. al., 2013; Ricoeur, 2000), which includes aspects of inconclusiveness and indexicality typical of the process of creation and criticism of knowledge (Parker, 2007). We are interested in pointing out that there are several investigations in the health sector that have focused on investigating the relationship/linkage of users with the health institution, opening the way to the study of accessibility centrally in its dimensions: geographical, economic, administrative, symbolic and relational (Stolkiner, A. et al 2008) by understanding that it constitutes a social practice of the system that underlies the guarantee of rights.
